Dr. Powell practiced in traditional “fee-for-service” medicine for 21 years in Grants Pass, OR. She was excited to learn about the concept of Direct Primary Care, believing it better serves the patient. This model allows the patient and doctor to develop a plan of care without bringing insurance companies into the decision-making process. She began the transition in 2019 and now offers personalized, quality medical care at Access Family Medicine.

Terms & Conditions of Membership:
Applicable enrollment fees are payable upon execution of this agreement.
In exchange for the services described herein, the member agrees to pay AFM the agree upon monthly fee. The required method of payment is recurring monthly debit or credit card charge, or bank account withdrawal. Membership fees will be charged monthly, and any other fees will be due at time of service
If this Agreement is terminated by either party before the end of an applicable monthly period, then AFM shall seek only partial payment for the final month of service based on the number of days of membership provided to the member for services rendered to the member up to the date of termination, as well as any outstanding miscellaneous charges.
Member(s) understands that both AFM and Member have the right to terminate this agreement at any time and for any reason. Such termination by either party must be in writing. The Member may terminate the agreement with one business day prior notice, but the AFM shall give thirty days prior written notice to the Member.
Please note the following:
- Access Family Medicine (AFM) as a practice is not insurance.
- In exchange for services, a member agrees to pay AFM a monthly debit card, credit card or bank withdrawal membership fee.
- The practice provides only the limited scope of primary care services specified in the retainer medical agreement.
- Any other fees will be due at time of service. The Member may terminate the agreement with one business day prior notice, but AFM shall give thirty days prior written notice to the Member. AFM shall seek only partial payment for the final month of service. Any refunds will be issued to Member(s) within three business days of stated termination date.
- A patient must pay for all services not specified in the retainer medical agreement.
- A complete list of primary services is included in the membership fee.
